BIRTH REGISTRATION PROCEDURES

One of the conditions for a newborn child to be registered for Vietnamese nationality is having one parent who is a Vietnamese national and the other being a foreigner who both agrees to choose Vietnamese nationality for their child in written “AGREEMENT ON CHILD’S NATIONALITY” agreeing to choose Vietnamese nationality for their child (click here to download the Form). The above-mentioned Agreement must be signed by both parents at the same time at the Consulate General, ​​in front of the Consulate General’s officer.

In case of not being presented at the Consulate General in person, the parents are requested to go to the Notary Public (Koso Yakuba) near their residence to certify their signatures on the above-mentioned Agreement which also requires the stamp seal by the Ministry of Foreign Affairs of Japan (MOFA) (sample here)

Parents should check with the nearby Notary Public/ Local Municipal Legal Affairs Bureau on how to get both signatures certification and authentication by Japanese MOFA, or can come directly to the Osaka Legal Affairs Bureau at this address (sample here).

In case parents have not applied for birth registration for the newborn child at any local/foreign competent authorities (e.g. Shiyakusho or Diplomatic Missions)

Procedure No.1:

I/ Application for Birth Certificate, Passport Issuance.

The Document shall include the following documents:

1/ Application form for birth certificate (download here), copy of birth certificate (download here). It is suggested that citizen should request for an additional copy of the birth certificate for multi-purposes usage in the future;

2/ Application form for Passport Issuance (download here); 02 color photos, white or blue background, size 3.5cm x 4.5cm (balanced photo, full face, not cropped).

3/ Original birth certificate sealed by a competent medical agency (出生証明書);

4/ Copy of Marriage Certificate;

5/ Original Certificate of Residence (Jyuminhyo) of parents;

6/ Copy of Parents' residence cards;

7/ Copy of page 2 & 3 of Parents’s Passports;

8/ Original Agreement on child’s nationality

9/ In case of applying for the above procedure by mail, it is advised to use Letterpack mail which has the tracking number (in order to track your documents/package easily)

A prepaid return envelope: if the applicant requests the visa be returned by mail, please use service of the delivery service company. A prepaid and self-addressed envelope must be enclosed with applicant’s passport, application form, visa fees and other current fees (if needed, such as visa approval arrangement fee, processing fee, shipping, handling and expedite services).

To avoid losses during delivery, it is advised to use certified mail such as Letterpack 520¥ (sample here)/ Yupakku (Sample here) with tracking number for your easy tracking.  The Consulate General shall not be responsible for any missing documents due to your choice of using uncertified mails.

I/ Application for the Excerpt of Birth Certificate, Passport Issuance.

1/ Application form for the Excerpt of birth certificate (download here), copy of birth certificate (download here). It is suggested that citizen should request for an additional copy of the birth certificate for multi-purposes usage in the future;

2/ Application form for Passport Issuance (download here); 02 color photos, white or blue background, size 3.5cm x 4.5cm (balanced photo, full face, not cropped).

3/ Original Document/Certificate of Acceptance of birth registration issued by the Competent Japanese Local Authority (Shiyakusho);

4/ Copy of Marriage Certificate;

5/ Original Certificate of Residence (Jyuminhyo) of parents;

6/ Copy of Parents' residence cards;

7/ Copy of page 2 & 3 of Parents’s Passports;

8/ Original Agreement on child’s nationality
